Social Care Networks in New York City: which network covers your borough and ZIP

New York City is not one Social Care Network but three — and eight ZIP codes in Brooklyn and Queens are assigned to the Staten Island network rather than the one covering the rest of their borough.

If you live in New York City and want to reach your Social Care Network, the first question is which one you are actually in. Three Lead Entities cover the five boroughs, and the boundaries are not purely borough-based.

The three NYC networks

The eight ZIP codes that break the pattern

This is the part worth checking before you call. Staten Island PPS covers eight ZIP codes outside Staten Island:

Brooklyn: 11209, 11219, 11220, 11228 — broadly Bay Ridge, Borough Park, Sunset Park and Dyker Heights.
Queens: 11355, 11368, 11373, 11377 — broadly Flushing, Corona, Elmhurst and Woodside.

If your ZIP is on that list, your network is Staten Island PPS even though you live in Brooklyn or Queens. If it is not, and you are in Brooklyn, Queens or Manhattan, your network is Public Health Solutions.

Neighborhood names are approximate. ZIP code boundaries do not match neighborhood boundaries exactly. Go by the ZIP code on your mail, not by what the area is called.

The Bronx

The entire Bronx is served by Somos Healthcare Providers, Inc. on 833-SOMOSNY (833-766-6769).

Just outside the city

If you are in the lower Hudson Valley or on Long Island rather than the five boroughs:

What to have ready when you call

Your Medicaid identification.
The name of your Medicaid managed care plan, if you have one.
Your ZIP code.
A short description of the unmet need — food, housing, transportation.
Any relevant diagnoses, and whether you are pregnant or postpartum.

You can also complete the self-screener on your regional network's website, speak with your provider or care manager, contact your health plan, or ask a participating community-based organization.

Language access

NYSDOH publishes member-facing materials in Spanish and Chinese, including the "Do I Have Medicaid" fact sheet and the Asthma Remediation fact sheet. Health plans commonly offer multilingual member services — Fidelis Care, for instance, describes its member line as multilingual and available 24/7. If you need an interpreter, say so at first contact.

Frequently asked questions

I live in Borough Park. Which network do I call?

Borough Park falls largely within ZIP 11219, which NYSDOH assigns to Staten Island Performing Provider System (917-830-1140). Confirm your exact ZIP code before calling, as neighborhood and ZIP boundaries differ.

Does Public Health Solutions cover all of Brooklyn?

No. Brooklyn ZIPs 11209, 11219, 11220 and 11228 are assigned to Staten Island PPS. The remainder of Brooklyn is covered by Public Health Solutions.

I am in the Bronx. Who covers me?

Somos Healthcare Providers, Inc., on 833-SOMOSNY (833-766-6769).

Are services the same in every borough?

The eligibility rules are statewide, but NYSDOH notes that service availability may vary by region. Your regional network can tell you what is currently available where you live.
