Asthma remediation under New York Medicaid: the exact criteria for home fixes

Asthma remediation is one of the most precisely defined services in the whole program. The criteria come down to counting: hospital stays, ER visits, urgent care visits, steroid courses and rescue inhaler prescriptions.

Asthma is the clearest case for treating housing as health care. Mold, dust, pests and poor ventilation drive attacks; attacks drive emergency room visits; emergency room visits cost Medicaid far more than fixing the apartment would. New York's Social Care Network program funds the fix.

The social risk factor

To be considered, a member must be assessed as having an unmet HRSN in the housing domain, require a clinically appropriate home modification or remediation service, and have a health condition that is exacerbated by their physical living environment.

The clinical criteria — this is the precise part

The member must be diagnosed with asthma, and meet at least one of the following:

• One or more hospital inpatient stays related to asthma within the last 12 months;
• Two or more emergency department visits related to asthma within the last 12 months;
• Three or more urgent care visits related to asthma within the last 12 months;
• Two or more prescribing events for oral steroid use related to an asthma diagnosis within the last 12 months; or
• Three to eleven prescribing events for a rescue inhaler, including albuterol, within the last 12 months.

What counts as the last 12 months

All five criteria are measured over the preceding 12 months. This means a member who had a bad year and has since stabilised may still qualify — and a member whose asthma is worsening now may need to wait until the pattern is documented. Your prescribing and visit history is the evidence.

What remediation actually covers

Asthma remediation sits within the housing services family. Related services that frequently matter for asthma include:

• 2.2a — mold and/or pest remediation;
• 2.2b — repairing or improving ventilation systems, including air conditioners, heaters, humidifiers and dehumidifiers. Note that asthma is named in the State's chronic condition list for this service too.

Getting the documentation right

Because the criteria are utilization-based, the supporting evidence usually already exists in your medical and pharmacy records. The practical steps:

1. Ask your provider to confirm the asthma diagnosis in your record.
2. Ask for a summary of asthma-related visits and prescriptions over the last 12 months.
3. Raise the housing conditions explicitly at screening — mold, pests, damp, no ventilation, no air conditioning.
4. Where the service requires it, obtain the provider attestation of medical necessity.

Children

High-risk children under the age of 18 are one of the enhanced population categories, which means a child with poorly controlled asthma living in poor housing conditions may be among the strongest candidates for this service. The same utilization criteria apply.

My child has asthma but has never been hospitalised. Do we qualify?

Possibly. Hospitalization is only one of five alternative criteria. Two or more asthma-related ER visits, three or more urgent care visits, two or more oral steroid prescribing events, or three to eleven rescue inhaler prescribing events in the last 12 months each satisfy the clinical test on their own.

Does the program fix mold in a rented apartment?

Mold remediation is a covered service where the criteria are met. Landlord obligations are a separate legal matter — the program does not remove your landlord's responsibilities, and tenancy sustaining services can link you to legal help.

How many inhaler refills is 'three to eleven prescribing events'?

The State counts prescribing events rather than canisters. Your pharmacy record is the source. If you are near the boundary, ask your Social Care Navigator to assess you against all five criteria.

Is an air purifier covered?

Service 2.2b refers to repairing or improving ventilation systems and names air conditioners, heaters, humidifiers and dehumidifiers, and the associated social risk factor language references an air filtration device. Whether a specific device is approved depends on clinical appropriateness in your case — ask your navigator.
